How to Fill out the Sprout HR Template

The Sprout HR Employee Information template is where new companies can bulk upload names of multiple employees along with their latest information into the Sprout HR system. While it is possible to do this as well via the Add Employee page on Sprout, the Employee Information Template saves a lot of time and effort in putting down all necessary employee information in one place for uploading en masse into the system.

The Employee information template can be downloaded under the Administration Tools tab, by selecting Bulk Upload from the drop-down list that appears. Let's get to know this template better.
First, make sure you've already set-up the following. Certain fields on the template rely on these prerequisites:
 

 Now here's how to fill out the template:

All items marked with an asterisk (*) are required.
 

A. Status after upload - leave this column blank. When you upload the template with incorrect data, an annotated excel template is automatically downloaded - and all the details for correction appear in this column.

B. *Employee ID No - Alphanumeric characters are accepted.

C. *Last Name

D. *First Name

E. Middle Name

F. *Gender 

G. *Date of Birth - Format is mm/dd/yyyy. Employees added via the HR template will have his or her birthday as the default password.

H. *Civil Status

I. Home Address

Format:
Unit & Floor No., Street, Building, Subdivision / Apartment / Village, Barangay, City, Province, Country

J. *Contact Number

Here are the accepted formats:

Mobile: 11 or 12 digits (eg. 09171234567 or 639123456789)
Landline: 7 or 9 digits (e.g. 8000000 or 028000000)
Home: 7 or 9 digits (e.g. 8000000 or 028000000)
Work: 7 or 9 digits (e.g. 8000000 or 028000000)
Other: 11 or 12 digits (eg. 09171234567 or 639123456789)

K. *Email This email will be used to receive email notifications such as attendance applications, Timesheet Lock, etc.

L.*Biometric ID - Input the biometric ID. Must contain numbers only with a maximum of 10 digits.

M. Employee Type - Select if Rank and File, Manager, or Officer.

N. Pay Group

O. Payroll Run Type - Select from Daily, Hourly, Semi-Monthly, and Monthly

P. Job Title

Q. *Department - Select a Department that has already been previously set up.

R. Cost Center - Select a Cost Center that has already been previously set up.

S. Location - Type in the exact title of the Workplace that has already been set up.

T. Sprout HR User Level - Select the access level to be assigned to the employee

U. *Immediate Supervisor’s Employee ID - Employee ID of the user's manager

V. *Employment Status - Select an employment status.

W. *Hire Date - Format is mm/dd/yyyy.

X. Regularization Date - Format is mm/dd/yyyy. 

Y. Status Date - Date of effectivity on Sprout Payroll

Z. Bank

AA. Bank Account Type - Select from the list of added banks

AB. Bank Account No.

AC. *TIN No. - Required.

AD. *Tax Status - Select from the dropdown list

 AE. OT Computation Table - Select from the dropdown list

AF. ROHQ

AG. Minimum Wage Earner

AH. Work Hours Per Day - Basis of the Hourly Rate computation

AI. Work Days Per Year - Factor rate for daily rate computation

AJ. Consultant Percent Tax

AK. Client Name

AL. Job Code

AM. Job Grade

AN. Billability

AO. Company Code

AP. ERPLN Code

AQ. Division

AR. Emergency Contact Name

AS. Emergency Contact Number

AT. Emergency Contact Relationship

AV. *Schedule Type

The following fields repeat from columns AX to CF for Monday to Sunday:  

AX. *Monday is Rest Day? (Yes/No) - Select Yes or No.

AY. *Monday Start Shift (HH:mm am/pm) 

AZ. *Monday Break In (HH:mm am/pm)

BA. *Monday Break Out (HH:mm am/pm)

BB. *Monday Start End (HH:mm am/pm) 

(fill out for Tuesday to Sunday until column CF)

CG. SSS No

CH. PhilHealth No

CI. PAG IBIG No

CJ. SSS Contribution - Select from the dropdown if there is a fixed amount

CK. PhilHealth Contribution - Select from the dropdown if there is a fixed amount

CL. HDMF Contribution - Select from the dropdown if there is a fixed amount

CM. HDMF Additional Contribution - if the employee has additional voluntary contributions

CN. PRC License No

CO. Passport No

CP. Basic Saalry

It may seem tedious here, but in reality, the Employee Information template is a nifty tool, and all in all easy to understand.
